The Passport Office: You have to specifically apply to the place where you applied for your first passport.
In my case, I had applied to mumbai, since I was born there. I scanned all my NORI documents. And send them by email- rpo.mumbai@mea.gov.in
This was the cover letter I wrote-

Respected Sir,
I am pursuing my postdoctoral research in the Department of Systems Biology, at the City
of. Hope National Medical Centre, USA. I am on J1 Exchange Visitor Visa status with two-year
home country residency requirement (Passport No. T3885074). However, the ongoing research
projects at current institute necessitates my presence and expertise for a longer period of time;
therefore, I would need to apply for a waiver from the USA homeland security.
As a part of application, I would need NORI from the Regional Passport Office (Thane,
India) where my first passport was issued. My passport was reissued in San Francisco, Indian
Embassy. I request you to kindly issue me a NORI/waiver certificate and forward it to the mailing
address. Also, please send the soft copy to my email. Please find the necessary enclosures to
facilitate the procedures.
